North Castle Ventures Publishes the AI-Native Medical Specification, Extending Sovereign Edge Compute to Clinical Real Estate
The vendor-neutral specification defines a zero-egress compute node built into the Medical Office Building, and is published under CC-BY-4.0 with a citable DOI alongside its AI-Native Office sibling.
North Castle Ventures has published AI-Native Medical: Sovereign Clinical AI Edge Infrastructure, a vendor-neutral technical specification that applies the sovereign compute thesis to clinical environments. Released as Draft Specification v0.2.0, the document is the healthcare sibling to the AI-Native Office specification and is stewarded on the same neutral footing — it names no preferred vendor, property or operator.
The specification addresses a specific operational bottleneck. Contemporary clinical workflows increasingly depend on continuous multimodal AI — ambient transcription, documentation synthesis, triage of high-resolution imaging — while routing Protected Health Information across public networks to hyperscale clouds introduces latency, recurring egress cost, and an expanded Business Associate Agreement and subprocessor surface.
In response, the document defines a healthcare real estate asset class: an on-premises sovereign edge compute node built into the Medical Office Building itself, running tenant-owned NVIDIA L40S-class GPU clusters in acoustically isolated, liquid-cooled utility enclaves. Ambient clinical data is ingested and processed locally rather than crossing the property network boundary, and the specification sets a measured glass-to-photon latency budget within 13 milliseconds for interactive guidance.
The specification is deliberate about the limits of that architecture. It positions zero egress as a physical and technical control within a defense-in-depth HIPAA Security Architecture pursuant to 45 CFR § 164.312 — not as a substitute for the administrative, physical and technical safeguards the rule requires. As the document states, architecture reduces the exposure surface; it does not by itself establish regulatory compliance.
Both specifications are published under a CC-BY-4.0 license with archived Zenodo records and citable DOIs, so institutional readers can reference a fixed version. For North Castle Ventures, stewarding two documents across commercial and clinical real estate reflects a single underlying position: as inference moves out of centralized cloud environments, the building becomes part of the infrastructure that governs where regulated data is allowed to travel.
